We require a referral from a healthcare provider prior to patients scheduling their appointment. Providers should make a referral through Epic, or by calling 513-636-4611, or by faxing a Specialty Services Form—available at the link below.

We understand it can be hard to wait for a referral. There are services available right away that can be started while you're waiting for your child to be seen.
