Each of these defects in the heart causes a bottleneck that makes it harder for blood to move forward. Blood can back up into the lungs and may not flow smoothly to the rest of the body.
The four defects are:
- Parachute mitral valve
- Supravalvar mitral ring
- Subaortic stenosis
- Coarctation of the aorta
Some children have all four defects, while others have only two or three. The combination and severity can vary. Early diagnosis and specialized care can help protect heart function and support healthy growth.

How the Heart Normally Circulates Blood
The heart is a strong muscle that pumps blood through the body. It consists of four chambers: two upper chambers (left atrium and right atrium) and two lower chambers (left ventricle and right ventricle). Blood travels from the lungs to the left side of the heart, which then pumps oxygen-rich blood to the rest of the body.
Several heart structures help control this flow:
- The left atrium receives oxygen-rich blood from the lungs.
- The mitral valve allows blood to move from the left atrium to the left ventricle.
- The left ventricle pumps blood out of the heart.
- The aortic valve opens so blood can leave the heart.
- The aorta (large blood vessel) carries this blood to the body.
When these structures are open and working well, blood moves smoothly through the heart. This steady flow allows the body to receive the oxygen it needs.
What Happens in Shone Complex?
Shone complex is a congenital left heart obstruction syndrome. This means several parts of the left side of the heart are narrow or blocked. These blockages slow the flow of blood leaving the heart, so the heart must pump harder to move blood through them. The different types of defects seen in Shone complex are:
Parachute Mitral Valve
In this condition, the mitral valve attaches to a single muscle inside the heart instead of two. This change limits how wide the valve can open. It may reduce blood flow into the left ventricle and cause backup of blood into the left atrium and the lungs, making it harder to breathe.
Supravalvar Mitral Ring
A thin ring of tissue forms above the mitral valve. This ring can narrow the opening and make it harder for blood to pass through the valve.
Subaortic Stenosis
Subaortic stenosis is a narrowing just below the aortic valve. This narrowing can be from extra muscle in the outflow of the left ventricle, a smaller-than-normal aortic valve, or a ridge or membrane of tissue that develops. This narrowing blocks blood as it leaves the left ventricle.
Coarctation of the Aorta
Coarctation of the aorta is a narrowing in the aorta itself. This limits how much blood can travel from the heart to the body.
Because several narrow areas can happen together, pressure inside the heart may increase. Symptoms depend on how severe each narrowing is.
