To request testing, download our test requisition form below. Notify our laboratory by phone at 513-803-3503 before sending samples, particularly if you are a new client, to ensure proper specimen collection and handling and to minimize the need for recollection.

The Hemostasis & Thrombosis Laboratory is a part of a comprehensive program that works closely with the clinical nephrology and molecular genetics laboratories to provide comprehensive testing for thrombotic microangiopathies (TMA), atypical hemolytic uremic syndrome (aHUS) and thrombotic thrombocytopenic purpura (TTP). Shipping Instructions for Assays
Laboratory Hours
The laboratory operates Monday through Friday, 8 am-4:30 pm. We cannot accept deliveries on Saturday or Sunday. We recommend the use of Federal Express. If another courier will be used, call the laboratory with the name of the courier and the tracking number of the package.
Shipping / Handling
Note all specimen collection, processing and shipping instructions on the requisitions. Package securely to avoid breakage and extreme weather conditions. Include a completed copy of our test requisition form with each sample.
