Improving Our Health System to Save Lives

At Cincinnati Children’s, we’re dedicated to eliminating youth suicide in the Greater Cincinnati Region. Our commitment to patient safety is unwavering because healthcare is about saving lives.

When it comes to patient safety, we agree that the only acceptable number of losses is zero. This life-saving mindset drives our efforts and aspirations with the Zero Suicide initiative—a core program within our Mental and Behavioral Health Institute (MBHI).

What is Zero Suicide?

Zero Suicide is a systems-level suicide prevention framework that has been implemented within the United States and across the world for nearly two decades. It’s also nationally recognized by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). This framework is comprised of patient-, provider-, environmental-, and systems-level interventions.

Zero Suicide has seven core tenets:

Lead

Lead systems-wide culture change committed to reducing suicide.

Train

Train a competent, confident and caring workforce.

Identify

Identify individuals with suicide risk via comprehensive screening and assessment tools.

Engage

Engage all individuals at risk of suicide using a suicide care management plan.

Treat

Treat suicidal thoughts and behaviors directly using evidence-based treatments.

Transition

Transition individuals through care with warm hand-offs and supportive contacts.

Improve

Improve policies and procedures through continuous quality improvement.

The Zero Suicide framework underscores the importance of learning at the system level, incorporating lived experiences, and fostering a restorative, just culture. It is driven by the belief that while we may not be able to predict who will die by suicide, we can still prevent it through a comprehensive and robust systems approach.

Why Zero Suicide at Cincinnati Children’s?

Unfortunately, suicide rates among youth within our area continue to increase despite significant allocation of mental health resources, with variability in evidence-based, suicide-focused training among providers. Zero Suicide gives us a standardized approach to improve—to understand, innovate and prevent suicide in our region. The framework helps us transform system-wide to provide safer suicide care and prevention.

As leaders in improving child health, we believe that Zero Suicide is a critical way for us to expand our leadership into the field of suicide prevention. And ultimately, a way to save the lives of the youth we serve.

Zero Suicide Implementation Team

An interdisciplinary Zero Suicide implementation team is essential for a complete approach to suicide prevention. By combining diverse expertise, teams can adapt strategies based on real-time data and empower champions to support implementation across our healthcare system and the community. Including individuals with lived experience enhances empathy and effectiveness, fostering a culture of commitment and accountability as we aspire to zero.

Our Zero Suicide implementation team brings together biostatisticians, county public health officials, data analysts, epidemiologists, grief care providers, medical records personnel, lived experience members, program managers, psychiatrists, psychologists, quality improvement specialists and social workers.
