Optimizing Access, Capacity and Flow

The Access Hub is the central capacity command center for optimizing system access, capacity and flow for every site of care throughout the health system. It serves as a single point of entry for Emergency Department referrals, hospital-to-hospital transfers, inpatient direct admits, and patient transportation. By integrating data analytics, real-time information, and communication tools, the Hub streamlines patient care processes and staff workflows.

How The Hub Works

The Access Hub is a centralized, physical location where patient care processes and staff workflows are monitored, managed, and optimized. Designed to function much like NASA Mission Control, the Hub provides a comprehensive, real-time view of operations across the health system. This visibility enables teams from multiple departments and specialties to make informed decisions, anticipate bottlenecks, allocate resources more efficiently, and proactively address patient-centric access challenges.

Built with employee comfort and high performance in mind, the Hub uses state–of–the–art technology to streamline access and support collaboration. Key teams and features include:

  • 100+ Staff Members based in the hub from the Admissions and Transfer Center, Transport Services, Environmental Services, Home Care, and Remote Patient Monitoring 
  • Critical daily partners include Financial Clearance (TCAuth), Nurse Helpline, Patient Services House Managers, and Inpatient Care Managers
