Your First Visit

Thank you for choosing Cincinnati Children’s for your child’s or your cancer care. Your first visit is a good opportunity for our team of specialists to get to know your child and their caregivers, better understand your child’s medical history and begin developing the best treatment plan. While here, you and your family will be treated with respect and compassion. We encourage you to ask questions and share information to help us best meet our patients’ needs.

Preparing for Your Visit

Our care team would like to learn as much about your child’s medical background as possible before your first visit. Please complete the medical history forms provided to you and bring them to your first appointment. These forms, along with records your referring physician sends to our team, will help us make the most of our time together. Be sure to include information about prescriptions, over-the-counter medications, vitamins and herbal and nutritional supplements the patient may be taking.
