Contact Us or Refer a Patient

For appointments, referrals and general information about the Tuberous Sclerosis Clinic at Cincinnati Children’s, contact 513-636-4222 or tsclinic@cchmc.org.

The International Patient Program can also be reached via online form to assist international users.

We accept physician referrals and self-referrals. Please contact your insurance provider to obtain authorization before self-referring.

Insurance Approval

We accept all forms of insurance and Medicaid. But unlike the process for a typical pediatric check-up, getting your insurance to cover specialty care requires a few additional steps. Complete the following to make sure care for you or your child is covered:

  • IMPORTANT: Call your health insurance provider and explain the visit.
  • Ask your insurance company if Cincinnati Children’s Hospital Medical Center is a provider. (We may also be listed as Children’s Hospital Medical Center of Cincinnati.)
  • Verify coverage and whether a referral from your or your child’s primary care physician is needed.
  • Additional documentation may be needed from other local providers.

If you or your child does not have insurance, or you are worried about being able to pay for care, we are here to help. Learn more about our financial assistance policy.

Test Results and Medical Records

If you have test results or medical records that are not from Cincinnati Children’s, you can mail or fax to:

April Hanifen, TSC Clinic Coordinator
TSC - MLC 2015
Cincinnati Children’s Hospital Medical Center
3333 Burnet Ave.
Cincinnati, OH 45229

Fax: 513-487-5585

It is important that we receive your or your child’s records and test results before the first visit. Also, let us know in advance if you or your child has special needs or concerns; we will do our best to accommodate them.

Online Second Opinions

We offer online second opinions for patients and families inside the United States who live beyond the Greater Cincinnati area (southwest Ohio, northern Kentucky and southeast Indiana).
