Self-Referrals
If you would like to self-refer, please contact your insurance provider to obtain authorization. We accept all forms of insurance and Medicaid. But unlike the process for a typical pediatric check-up, getting your insurance to cover specialty care requires a few additional steps. Learn more about confirming your insurance coverage.
Physician Referrals

Test Results and Medical Records
If you have test results or medical records that are not from Cincinnati Children’s, you can fax them to 513-487-5585 or mail them to:
