How to Become a Supplier at Cincinnati Children’s
To be considered as a new supplier at Cincinnati Children's, a New Supplier Request Form must be signed and submitted by a Cincinnati Children's employee to the Purchasing department.
If approved, domestic suppliers must enroll on the Taulia portal to provide tax identification forms and banking information. During enrollment, the supplier will be required to include the following:
- ACH Form
- Acceptance of Cincinnati Children's payment terms
Foreign suppliers will not register in the Taulia portal but will be required to submit W-8BEN.
Upon verification of all appropriate documentation, some suppliers may require annual health screening and immunization documentation if their services will be performed at the hospital. They will receive a badge for identification and security purposes. All suppliers will receive an email detailing these enrollment instructions.
