MyChart

MyChart – An Online Tool for Patients

 

Sign In to MyChart

MyChart allows you to schedule follow-up visits with care providers and gives you access to your child's medical records, allowing you to view test results, a list of medications, appointments, follow-up instructions, diagnoses and more.

MyChart can be accessed via the MyChart website or mobile app (available for download from the iTunes App Store and Google Play).

Sign Up for MyChart

Online

If your child is 17 years old or younger or you are 13 years old or older, you may now go online to request a MyChart account. Simply select the form that best describes you. A valid photo ID is required to sign up online, so look for the secure upload to submit your proof of identification. Allow seven business days when requesting a MyChart account online.

In Person

You can ask a member of the registration staff at your clinic or neighborhood location or visit the Family Resource Center in person to register for an account. You do not need an appointment.

MyChart should not be used for emergencies or urgent needs. Please call 911 or go to an emergency room if you are experiencing a medical emergency.

  • Signing Up for MyChart: MyChart accounts can be requested in person or online. For security purposes, MyChart accounts cannot be requested or created over the phone. When requesting a new account online, please allow up to 7 business days to receive your account activation email. Visit the How do I sign up for MyChart? section of the MyChart FAQ for details.
  • Test Results: Please contact your/your child’s provider if you need help with the release or interpretation of test results.
  • Missing or Incorrect Information: MyChart information comes directly from Cincinnati Children's electronic medical record. If you see something in MyChart you think is not correct, please contact Health Information Management (HIM) Monday through Friday, between 8:30 am and 5 pm.

    • MyChart: Click the "submit a chart correction request" link from the page with the incorrect information.
  • Billing: Billing Customer Service can help with any billing questions, including concerns over information that may not be available to view in MyChart. Representatives are available Monday through Friday, between 7:30 am and 5:00 pm.
    • Phone: 513-636-4427, option 9, or toll-free at 1-800-344-2462, option 9, ext. 4427.
    • MyChart: Select the MyChart menu, then select Ask a Billing Question under the Billing topic and complete the form.
  • Adding Providers to Care Team: If you would like a provider added to your/your child’s Care Team in MyChart, please contact a member of your Cincinnati Children’s health care team. MyChart analysts are not able to update this information for the provider.
