Understanding the Mechanisms Behind Chronic Pain
At Cincinnati Children's, the Pediatric Pain Research Center brings together clinicians and researchers from anesthesiology, neurology, behavioral medicine, gastroenterology and other specialties to advance understanding of pediatric chronic pain. "With the Pediatric Pain Research Center, we seek to understand pain both at very mechanistic levels as well as at very human levels. With the close integration of basic scientists, translational scientists, and clinician scientists, we have the ability to relate the disruptions of the nervous system that are associated with pain to the tremendous impact that they have on the individual and their families," says Robert Coghill, PhD, director of the Pediatric Pain Research Center.
A major example is BRIDGE (Biopsychosocial Risk Influences on the Development and Generation of Early-Onset Chronic Pain), a multidisciplinary study using biological, psychological and social data to better understand the mechanisms that contribute to chronic musculoskeletal pain in children.
This approach is reshaping how research is conducted. Investigators are studying differences in inflammatory signaling, neural activity and other biological factors across patient populations to better understand why children with similar symptoms may experience pain differently and respond differently to treatment.
"Historically, we have grouped patients based on a diagnosis or where their pain occurs. BRIDGE starts from a different premise: children with similar symptoms may have very different underlying mechanisms contributing to their pain. Understanding those differences is critical if we want to deliver more effective, individualized care," says Christopher King, PhD, of Cincinnati Children’s Division of Behavioral Medicine and Clinical Psychology.
"We anticipate that BRIDGE will move us beyond focusing on the patient's diagnosis and toward a future where treatment decisions are informed by a deeper understanding of the biological and psychosocial processes contributing to their pain. That has the potential to improve outcomes, reduce unnecessary treatments, and make the journey easier for families," King says.
