Becoming a Patient
The Cincinnati Children’s Cancer Survivorship Center accepts referrals from families and physicians. You or your child can establish care with our program if you meet the following criteria:
- You were diagnosed with pediatric cancer at least five years ago.
- You completed treatment for your pediatric cancer at least two years ago.
You do not need a physician referral to schedule an appointment at the Cancer Survivorship Center. We see many patients who “self-refer.” However, your health insurance plan may have rules about where you can seek care. Click here to learn more about health insurance.

Before setting up the first appointment, our experienced oncology nurses work with families and referring physicians to obtain a detailed medical history, copies of laboratory and radiology reports and other helpful materials. They create a summary for the patient to take home as a helpful reference.
