Becoming a Patient
The Cincinnati Children’s Cancer Survivorship Center accepts referrals from families and physicians. You or your child can establish care with our program if you meet the following criteria:
- You were diagnosed with pediatric cancer at least five years ago.
- You completed treatment for your pediatric cancer at least two years ago.
You do not need a physician referral to schedule an appointment at the Cancer Survivorship Center. We see many patients who “self-refer.” However, your health insurance plan may have rules about where you can seek care. Click here to learn more about health insurance.

For Patients and Families: Please contact the Cancer Survivorship Center through our online form below, or by calling 513-517-2234. For form inquiries, a staff member will contact you within two business days.
The International Patient Program can also be reached via online form to assist international users.
For Physicians: Please contact the Physician Priority Link®, 888-636-7997 or 513-636-7997, for patient referrals or if you’d like an expert from the center to provide a consultation or second opinion.
