Home Care Specialty Pharmacy

Our pediatric-trained specialty pharmacists understand chronic and rare conditions. They know the effect these conditions can have on children and their quality of life. Specializing in self-injected or self-infused medications, our pharmacists provide you with the information you need about your child’s medication. They monitor the effectiveness and safety of your child’s medications and work closely with your child’s doctors and nurses to coordinate the best care for your child.

Why a Specialty Pharmacy

Specialty medications are more complex and are not available through your local retail pharmacy store. They require additional education to administer and close monitoring of side effects. Specialty medications can be expensive and may require special insurance approval.

Patient Management Program: a Collaborative, Coordinated Approach to Care

Close monitoring of your child’s specialty medication is an important part of improving your child’s health. To provide a higher level of personalized care, we will enroll your child in a patient management program specific to their diagnosis. We are committed to helping you manage your child’s chronic condition in the comfort of your own home. This program allows our pharmacy team to give you the support you need to successfully manage your child’s care at home.

Patient Management Program services include:

  • Education on prevention and management of medication side effects
  • Refill reminders
  • Checking insurance benefits to make sure your child’s medication and supplies are covered
  • Connecting you with resources, such as travel kits, and tools to help you keep track of your child’s medication
  • Information about your child’s health condition, diagnosis, and related tests and procedures
  • Regular pharmacy follow-up to:
    • monitor medication usage
    • address side effects and medication concerns
    • review your child’s medications
    • assess your child's response to treatment
